By creating an account on the CoreSignal platform (the "Platform") or using any CoreSignal services, you (the "User") agree to be bound by these Terms of Service (the "Terms"). If you do not agree, do not use the Platform. These Terms constitute a legally binding agreement between you and CoreSignal, Inc. ("CoreSignal").
CoreSignal is a lab-based men's health optimization tool that helps you organize self-reported health information, lab values, and wearable data, generates an informational Optimization Index score, and surfaces AI-generated insights. The Platform is a wellness optimization tool, not a clinical diagnostic platform. It is provided for informational and educational purposes only and does not constitute medical advice, diagnosis, or treatment. Using CoreSignal does not create a physician–patient relationship.
You must be at least 18 years of age to use the Platform. By accepting these Terms, you represent that you meet this requirement and have the legal capacity to enter into a binding agreement.
By using the Platform, you provide the following explicit, informed, and granular consent to the collection and use of your health data:
You may withdraw any of these consents at any time by contacting braiden@coresignal.health. Withdrawal will not affect the lawfulness of processing prior to withdrawal.
CoreSignal is currently offered as a closed pilot to a limited group of participants. Access may be granted by invitation. During the pilot, features may change, be suspended, or be discontinued at any time, and we may terminate or limit access without notice. The Platform is provided "as is" during the pilot and may contain errors or incomplete functionality. Feedback you provide during the pilot may be used to improve the Platform.
CoreSignal may offer paid subscription tiers that unlock additional features. Subscription fees, billing intervals, and any applicable trial terms are presented at the point of purchase. Subscriptions renew automatically at the end of each billing period unless cancelled before the renewal date. You may cancel at any time; cancellation takes effect at the end of the current billing period and does not entitle you to a refund for the current period except where required by law. Pilot participants may receive promotional or complimentary access, which may be modified or withdrawn at our discretion.
CoreSignal Health is a wellness data aggregation and analytics platform. It is not a medical device as defined under 21 U.S.C. §321(h), has not been cleared or approved by the U.S. Food and Drug Administration, and is not intended to diagnose, cure, treat, prevent, or mitigate any disease or medical condition. Scores, flags, and insights generated by the Platform are for general wellness informational purposes only and do not constitute medical advice, clinical decision support directed to end users, or a substitute for the professional judgment of a licensed healthcare provider.
Certain Platform features surface informational flags when user-entered or connected lab data falls outside reference ranges used in the Platform. These flags are for general wellness awareness only. They do not constitute diagnoses, clinical assessments, or medical recommendations. Reference ranges displayed are general population reference ranges and may not be appropriate for your individual health status, age, medications, or clinical history. Always consult a qualified, licensed healthcare professional before making any health or treatment decision.
The CoreSignal Provider Portal presents aggregated patient wellness data and informational flags to licensed healthcare professionals ("HCPs") for informational purposes. These outputs are intended to supplement, not replace, the independent clinical judgment of the reviewing HCP. CoreSignal does not employ licensed healthcare professionals and does not provide clinical services. The HCP is solely responsible for all clinical decisions made in connection with patient care. Nothing in the Platform constitutes a recommendation that an HCP take or refrain from any specific clinical action.
The Platform is operated from Alabama and governed by Alabama law. CoreSignal does not provide medical services, practice medicine, or employ licensed physicians in any jurisdiction. Clinic partners using the Provider Portal are solely responsible for compliance with applicable state law, including corporate practice of medicine requirements in their respective states of operation. CoreSignal's flat-fee software services are provided at arm's length to clinic management entities and are not contingent on clinical revenue or patient volume.
Any supplement, lifestyle, or wellness protocol suggestions provided through the Platform are general wellness recommendations based on publicly available, peer-reviewed literature. They are not medical prescriptions, clinical treatment recommendations, or replacements for physician guidance. Individual results vary. Always consult a healthcare provider before starting any supplement or wellness protocol, particularly if you have existing medical conditions or take prescription medications.

CoreSignal will not use your individually-identified health data to train external AI models without explicit additional consent.
The Platform, including its software, algorithms, scoring methodology, and content, is the exclusive property of CoreSignal, Inc. Your health data remains your property. You grant CoreSignal a limited, non-exclusive license to process your data as described in these Terms and the Privacy Policy.
Your use of the Platform is also governed by our Privacy Policy, which describes what data we collect, how we use it, the vendors that process it under Business Associate Agreements, and your rights regarding your information. By using the Platform, you acknowledge and agree to the practices described in the Privacy Policy.
To the maximum extent permitted by applicable law:
CoreSignal's total liability to you for any claim arising out of or related to these Terms or the Platform shall not exceed the greater of (a) the total fees paid by you to CoreSignal in the 12 months preceding the claim, or (b) one hundred dollars ($100).
CoreSignal shall not be liable for any indirect, incidental, special, consequential, or punitive damages, including loss of data, loss of profits, bodily injury, or personal injury, even if CoreSignal has been advised of the possibility of such damages.
Some jurisdictions do not allow the exclusion or limitation of certain damages. In such jurisdictions, CoreSignal's liability is limited to the maximum extent permitted by law. The Platform is provided without warranties of any kind, whether express or implied. You use CoreSignal at your own risk.
You agree to indemnify, defend, and hold harmless CoreSignal, its officers, directors, employees, and agents from any claims, liabilities, damages, losses, and expenses (including reasonable legal fees) arising out of your use of the Platform, violation of these Terms, or violation of any third-party rights.
CoreSignal may terminate or suspend your account for violation of these Terms. You may delete your account at any time. Upon deletion, data will be retained for 3 years per the Privacy Policy retention schedule, then securely deleted.
These Terms are governed by and construed in accordance with the laws of the State of Alabama, without regard to conflict of law principles. Any disputes arising under these Terms shall be resolved in the state or federal courts located in Alabama. You consent to the exclusive jurisdiction of such courts.
Before initiating formal dispute proceedings, you agree to attempt to resolve disputes informally by contacting braiden@coresignal.health.
CoreSignal may modify these Terms at any time. Material changes will be communicated via email and Platform notice at least 30 days before taking effect. Continued use of the Platform after the effective date constitutes acceptance of the updated Terms.
